Treatment records (Civil Code § 630f)
Whoever provides treatment must keep a treatment record on paper or electronically, close in time to the treatment, recording the history, diagnoses, examinations and their results, findings, therapies and their effects, interventions, consent and information given. Corrections must leave the original content and the time of the change visible, also in electronic records. The record is kept for ten years after the treatment ends, unless other rules set other periods.
